Agreement *Participant Authorization: I/We, the undersigned, certify that I/We am/are the parent(s) or legal guardian(s) of the participant enrolled in the Euro Pro Id Camps. 1. Consent for Medical Treatment I/We give permission to Camp staff to authorize and obtain necessary medical treatment for the participant. I/We accept responsibility for all associated medical costs. 2. Athletic Release & Assumption of Risk I/We confirm the participant is physically able to engage in camp activities. I/We understand and assume all risks involved in soccer activities and release Euro Pro Pathway and IPC, and all associated staff from any liability for injury or loss, whether or not due to negligence. No refunds will be issued due to injury during the Camp. 3. Media Release I/We authorize Euro Pro Pathway and IPC,. to use images and video of the participant for promotional purposes. 4. Weather Policy In case of inclement weather, campers will be taken to a designated shelter. If camp is canceled after starting, guardians must pick up the participant 5. Cancellation & Refund Policy Cancellation requests must be submitted in writing via email to info@europropathway.com . The refund policy is as follows: • Refund request received 30 days or more before ID camp date a refund of 50% will be issued • No Refunds will be for refund request received less than 30 days before ID camp • No-show: No refund If the Camp is canceled by Euro Pro Pathway or IPC full refunds will be issued within 30 days, or participants may transfer to another camp, subject to availability. Our liability is limited to the refund of fees paid. No refunds are issued once camp is in progress or due to circumstances beyond our control. 6. Force Majeure / Acts of God In cases where Euro Pro Pathway cannot fulfill obligations due to external factors (e.g., pandemics, extreme weather), no refund is guaranteed.
